Understanding Dengue Fever in India

Dengue fever is a mosquito-borne viral infection caused by four distinct dengue virus serotypes (DEN-1, DEN-2, DEN-3, and DEN-4). Transmitted primarily by female Aedes aegypti mosquitoes, dengue poses a major public health challenge during monsoon and post-monsoon months across India. Early symptom recognition and timely fluid management are vital to prevent progression to severe dengue hemorrhagic fever (DHF) or dengue shock syndrome (DSS).

Early Symptoms of Dengue Fever

Symptoms typically manifest 4 to 10 days following an infective mosquito bite and persist for 2 to 7 days. Key clinical manifestations include:

• Sudden High Fever: Temperature spiking up to 104?F (40?C).
• Retro-Orbital Pain: Severe pain located behind the eyes, exacerbated by eye movement.
• Severe Myalgia and Arthralgia: Intense muscle and joint pain, colloquially termed "breakbone fever."
• Severe Frontal Headache: Throbbing headache across the forehead and temples.
• Maculopapular Rash: Red skin rash appearing 2 to 5 days after fever onset.
• Nausea and Vomiting: Gastrointestinal distress accompanied by reduced appetite.

Critical Warning Signs of Severe Dengue

As the initial fever abates (usually between days 3 and 7), patients must be closely monitored for plasma leakage signs. Seek immediate emergency medical care at an ICU or tertiary hospital if any of the following warning signs occur:

• Severe, persistent abdominal pain or tenderness.
• Persistent vomiting (at least 3 episodes in 24 hours).
• Mucosal bleeding (epistaxis, bleeding gums, blood in vomitus or stool).
• Lethargy, extreme restlessness, or sudden drop in body temperature below 36?C (96.8?F).
• Rapid decline in blood platelet count accompanied by rising hematocrit (hemoconcentration).
• Fluid accumulation causing breathlessness or pleural effusion.

Diagnostic Protocols and Laboratory Tests

Accurate clinical diagnosis involves combined serological and hematological testing:

1. Dengue NS1 Antigen Test: Recommended during the first 1 to 5 days of fever onset to detect viral non-structural protein 1.
2. Dengue IgM / IgG Antibody Test: Detects specific immunoglobulins from day 5 onwards to confirm acute or secondary infection.
3. Complete Blood Count (CBC): Serial testing of platelet count and hematocrit levels every 12 to 24 hours to monitor plasma leakage.

Clinical Treatment and Home Care Guidelines

There is no specific direct antiviral medication for dengue virus. Clinical management focuses on supportive care and strict fluid therapy:

• Oral Rehydration: Consume plentiful fluids including Oral Rehydration Salts (ORS), fresh coconut water, clear soups, and water.
• Fever Management: Administer Paracetamol (Acetaminophen) as prescribed by a medical doctor. AVOID Aspirin, Ibuprofen, and NSAIDs as they aggravate bleeding risks.
• Rest: Strict bed rest during the febrile and critical recovery phases.
• Hospitalization: Inpatient admission is mandatory for patients exhibiting warning signs, low platelet counts (<50,000/?L), or severe fluid loss.

Prevention and Mosquito Vector Control

Eliminating mosquito breeding grounds remains the most effective defense against dengue transmission:

• Clear standing water from air coolers, flower pots, tires, and water storage drums weekly.
• Apply DEET or Icaridin-based insect repellents on exposed skin.
• Wear long-sleeved clothing and long trousers, especially during dawn and dusk when Aedes mosquitoes feed.
• Utilize window mosquito screens and bed nets treated with insecticide.
